OUR PLEDGE TO THE COMMUNITY
Raphael Health Center pledges to pursue its foundational mission: providing high quality health services to all community members that reflects a racially equitable standard of care. Sharing the love and healing of Jesus Christ informs and calls Raphael to provide respect and dignity in a culturally sensitive manner. More than just words, our pledge will drive our long term strategy, our daily internal work with patients and partners at the Center, as well as our external community relationships.
PLEDGE 1
Raphael Health Center pledges to provide healthcare that seeks to not only understand, but correct racial inequities and unjust healthcare services.
Focus Areas: Health
This will mean that all healthcare services offered at the Center will be provided based on the consideration of how to increase and enhance access to preventative, diagnostic and treatment services that is both just and equitable.
2022 PROGRESS
Raphael has developed healthcare measures that have specific focus on Diabetes Control; Hypertension; and Childhood Immunizations. Rates are reviewed for Asian, African American, Hispanic and Caucasian Patients. Currently we are working on developing a plan to increase the rate of Childhood Immunizations for African Americans. This rate is markedly lower than all other populations we review.
2021 PROGRESS
Raphael includes racial and ethnic patient care outcome targets in its quality access measures, which include preventative screening measures and chronic disease management goals. It holds itself accountable to continue to pursue those measures specific to the best practice guidance and process improvement plans. The Center's Quality Improvement/Assurance Team monitors this on a monthly basis.

Raphael Health Center pledges to partner with community members, services, and programs to reduce health disparities and promote healthcare equity as the basis to transform where we live, work and play.
Focus Areas: Health and Prosperity
We commit to developing and growing meaningful relationships in the community that provide an integrated and purposeful approach to healthcare that reduces disparities and grows just, equitable healthcare for all.
2022 PROGRESS
Raphael is collaborating with StepUP, Pathways Recovery, Salvation Army, Dove House, and Fathers and Families to support efforts made by those key partners and facilitate services between and among each of them.
2021 PROGRESS
Raphael has, and will continue to, grow and maintain relationships that support integration and facilitation of efforts to reduce health disparities and promote healthcare equity. A few of these include: The Neighborhood Christian Legal Clinic; R.E.D. Alliance; Pathways, etc.

Raphael Health Center pledges to develop and enhance a work place that reflects the community it serves and demonstrates respect and value for all team members.
Focus Areas: Health, Prosperity, and Education
We will invigorate our Health Center work team by promoting a diverse, equitable and just work team that reflects the community in which we serve.
2022 PROGRESS
Raphael has a diverse workforce, but is making efforts to assure that Leadership and Provider positions reflect the population we serve. This is not only important for our internal team, but also for the trust and respect of our community members and neighbors.
2021 PROGRESS
Raphael continues to develop its work team with a focus on a diverse work force that reflects our patient population and the community we serve. This includes racial minority inclusion, ethnic minority inclusion, especially those who do not speak English as their 1st or primary language. Raphael also pursues a Board of Directors that reflects the community in which it resides and for whom it serves.
